Patient Referral
When referring a patient to the practice please include all information requested on our standard referral form including current patient contact telephone numbers.
Please note, we request that all referrals are accompanied by relevant radiographs/scans and where possible are delivered to our city practice prior to the patient’s consultation appointment.
To schedule the most appropriate appointment for your patient we would appreciate receiving your referral in advance and we will then contact your patient to book a convenient consultation appointment.
